AND USE CONCERNS
The UCSF
Institutional Animal Care and Use Committee (IACUC)
has approved the following
instructions and procedures for reporting and following
up on the reporting of all animal care and use concerns.
Reports can be submitted on the “
UCSF Animal Care and Use Reporting Form”. Individuals
may also report their concerns by phone, in writing, or
by fax or e-mail if confidentiality is not
an issue (see specific information below, under "Individual
Reporting Concern").
